Step 1: Call the HART office at 707-725-9381 to schedule an intake / assessment appointment.

Step 2: During your scheduling phone call we will go over all of the documents you will need to mail in/bring with you, the forms you will need to sign, and the cost of the program that the court has appointed for you.

Here is what you will need for enrollment :

•  A list of current medications you are taking, if any

• Most recent violation date

• Court docket or case number, conviction date and state of arrest

• Program length required by court or DMV for license reinstatement

It is your responsibility to provide us this information. No proof of enrollment can be provided without all of your required information.

We will also provide you with any additional contact info, our address, and any online links you may need to complete the program. Be sure to write this information down.

 If you arrive late for your appointment or do not have all the items you were asked to bring, you will be asked to reschedule your appointment. This guarantees that we have all the necessary information needed to get you set up with the proper program.

Step 3: Attend your appointment and bring all necessary documentation, please keep in mind that you will need to select a reoccurring day and time for your HART classes, and that you must always be available for those meetings.
